SUPPLIER AND SUPPLIER EMPLOYEE/AUTHORIZED PERSON INFORMATION NOTICE

This information notice has been prepared by ÜNLÜ AKADEMİ DENTAL POLYCLINIC JOINT STOCK COMPANY (“Company”), acting as the data controller, within the scope of Article 10 of the Law No. 6698 on the Protection of Personal Data and the Communiqué on the Procedures and Principles to be Followed in Fulfilling the Obligation to Inform.

The Company aims to process personal data belonging to its suppliers in accordance with the Law No. 6698 on the Protection of Personal Data (“KVK Law”) and other relevant legislation. With this Information Notice, it is adopted to maintain and improve the activities carried out by the Company in compliance with the principles set forth in the KVK Law.

The personal data collected and used by our Company regarding suppliers mainly include the following:

Identity Data

Name-Surname, Turkish Identification Number, Signature

 

Contact Data

Telephone Number, Address, E-mail Address

 

Financial Data

Bank Details, IBAN Information, Amount Information

The above-mentioned personal data are processed in all circumstances in accordance with the law and the principles of good faith, by maintaining the accuracy and up-to-dateness of the personal data you share, for specific, explicit and lawful purposes, and in a manner that is relevant, limited and proportionate to the purpose for which they are processed.

Purposes of Processing Personal Data:

Your personal data are processed by our Company, limited to the purposes of Compliance with Legislation, Execution of Finance and Accounting Processes, Execution of Goods/Services Sales Processes, Execution of Goods/Services Production and Operational Processes, Execution of Goods/Services Procurement Processes, and Execution of Communication Activities.

To Whom and For What Purpose the Processed Personal Data May Be Transferred:

Your personal data collected during this process may be transferred, to the extent permitted by the provisions of the Labor Law, Occupational Health and Safety Law, Social Insurances and General Health Insurance Law, Law on the Regulation of Publications on the Internet and Combating Crimes Committed Through Such Publications, Turkish Commercial Code, Tax Procedure Law, Law No. 6698 on the Protection of Personal Data and other relevant legislation, to authorized institutions or organizations; to public legal entities such as the Personal Data Protection Authority, Ministry of Treasury and Finance, Ministry of Trade, Ministry of Labor and Social Security, Information and Communication Technologies Authority; to our subsidiaries and/or direct/indirect affiliates; and to real persons or private law legal entities with whom we have contractual relationships, cooperate, or receive services from in order to carry out our activities as ÜNLÜ AKADEMİ DENTAL POLYCLINIC JOINT STOCK COMPANY, and who are jointly and severally responsible with us for taking workplace security measures such as the protection of your personal data, prevention of unauthorized access, and prevention of unlawful processing.

Method and Legal Basis of Personal Data Collection:

Your personal data are collected in physical and electronic environments for the purposes specified above based on the legal grounds stated in Articles 5 and 6 of the KVKK, namely being mandatory for the data controller to fulfill its legal obligation and being explicitly stipulated in laws. Your personal data will be retained for the reasonable period specified in the relevant legislation or until the purpose of processing ceases to exist, and in any case, for the statutory limitation periods.

  1. Rights of the Supplier

Pursuant to Article 11 of the KVK Law, you may apply to our Company and submit requests regarding the following matters concerning your personal data:

  1. Learning whether personal data are processed,
  2. Requesting information if personal data have been processed,
  3. Learning the purpose of processing personal data and whether they are used in accordance with this purpose,
  4. Knowing the third parties to whom personal data are transferred domestically or abroad,
  5. Requesting the correction of personal data if they are incomplete or inaccurately processed and requesting notification of the transaction carried out within this scope to third parties to whom personal data are transferred,
  6. Requesting the deletion or destruction of personal data if the reasons requiring processing cease to exist, despite having been processed in accordance with Law No. 6698 and other relevant laws, and requesting notification of the transaction carried out within this scope to third parties to whom personal data are transferred,
  7. Objecting to the occurrence of a result against the data subject by analyzing processed personal data exclusively through automated systems,
  8. Requesting compensation for damages in case of suffering damage due to unlawful processing of personal data.

Method of Submitting Requests:

You may submit your requests within the scope of Article 11 of the Law, which regulates the rights of the data subject, in accordance with the “Communiqué on the Procedures and Principles of Application to the Data Controller,” by filling out and signing the application form available on the Company’s website and delivering it in writing to our Company’s address at SIRAKAPILAR MAH. 506. SK. NO:12 MERKEZEFENDİ/DENİZLİ in person with proof of identity or via a notary public, or by sending it via the e-mail address verified for your membership or through your Registered Electronic Mail (KEP) address. Depending on the nature of your request, your application will be concluded free of charge as soon as possible and within a maximum of 30 (thirty) days. However, if the process requires additional costs, a fee may be requested from you according to the tariff to be determined by the Personal Data Protection Board. If the response exceeds 10 (ten) pages, a processing fee of 1.00 TL will be charged for each page. If the response is requested to be provided on a recording medium such as a CD or flash drive, a fee may be requested according to the cost of the requested medium.

Amendments:

Amendments may be made to this information notice depending on legislative changes. In case of any amendment, necessary announcements will be made on our website, and it is recommended that you regularly visit our website to stay informed of such changes.
